This page is probably slow loading for most people.  However for the greatest clarity and printability possible I have presented it in this form.

Below is an actual consent form that is used for the general public to grant Kaiser permission to allow nurses to perform biopsies on a patient's colon. The patient seldom knows who will actually be doing the biopsy until they are sedated and unable to do much to stop the procedure anyway.  Personally if I were to be in a situation where I had to allow someone with a sharp instrument to scrape the insides of my gut I think that I would prefer to have a licensed medical doctor do it.  I do take issue with the statement in this document that says: "It should not hurt as there are no pain nerves in the lining of the intestines."  That is not completely true.  It can very well hurt a great deal but the truth is that is not likely to happen.
